The GPU might have or not been damaged by the PSU that is what was implied. Like your GPU went kaput 3 times in a row within 5 years which makes it likely that the PSU's voltage regulation wasn't good. And it is enforced by the fact that you used a Corsair VS series PSU which is a low quality PSU for a card like GTX 970. That's why we suggested to upgrade the PSU first. You are just lucky that you are still within the warranty period for the GPU.
Sorry for the late reply. Got corsair rmx PSU, gpu same condition. Gtx gpu has been sent for RMA, yet to receive the replacement (at present, out of stock, hence delay).
Will update once I receive the gpu.
